All students are required to complete a student folder and these additional forms. Make time to stop by the shop to fill out the student folder and forms.
Read through and fill out the Diver Medical form first. This will help you determine if you need a doctor's signature to participate in the course.
You can either fill out the additional forms at home and turn them in at the shop, or fill them out while in the shop.

Continuing education forms
Stop by the shop before your course to turn in or fill out the required paperwork. You can either fill out these forms at home and turn them in at the shop or fill them out while in the shop. If there are additional course-specific forms, they will be provided when you visit the shop to fill out your student folder and purchase gear, or at the time of the class.
