All students are required to complete a student folder and these additional forms. Students must also pick up their course manual to complete the required reading before their camp session.

Students can pick up their course manual starting May 1.

Make time to stop by the shop to fill out the student folder.

You can either fill out the additional forms at home and turn them in at the shop, or fill them out while in the shop.

Diver Medical

Read the Diver Medical Form carefully to determine if you need a physician's approval to participate in this course.
